Our team holds a high star rating, and we can begin care within 24 to 48 hours. This is important as it closes time gaps in the continuum of care. We provide specialized disease management programs and services are 100 percent covered by Medicare. We also take most insurances.
With an easy referral process, we admit patients seven days a week, 24 hours a day. We provide live on-call support year round, with outstanding customer care 24 hours a day. Community Liaisons are available to help with the discharge process and to educate patients and Families. We can help with agency standard of referral to admissions within 24 hours of discharge.
We are chronic disease management experts and promote education, independence, and self-care to the individuals we serve. Specialized health therapy services can be provided through our clinical pathways programs. These pathways include wound care, IV infusion, dementia and Alzheimer care, and we will create care plans customized to every patient’s needs.
Home health educational services are available if needed for family members. Care Coordination can be made via patient conference on a bi-monthly basis. Primary care physician follow-up can be made within 72 hours of admission. We can provide continued communication and follow-up with healthcare providers, throughout the plan of care.
